This plaque reads (in German):

Here lived Leopold Tomschik
Born 1903
Arrested 4.2.1942
Penitentiary Brandenburg
Flight into death 17.3.1944

It’s part of a whole series of ‘Stolpersteine’ – Stumbling Stones – that dot the city of Berlin to commemorate individual victims of NAZI atrocities. They are quietly placed in amongst the cobble stones on the street outside the houses where the victims lived, and started as a private project without official permission. This particular one is located outside Dunckerstraße 58 in Prenzlauer Berg and was laid on the 13.9.2008, according to the excellent stolpersteine-berlin.de website. Although the website already documents the Stolpersteine extensively, I was struck by their solemn grace and think they deserve to be seen as widely as possible.
